Program Participants:
- Have the ability to communicate and work in a team,
- Are independent in their self help skills,
- Can benefit from support in accessing the community,
- Live at home with their families or semi independently in the community,
- Are in transition from school to post age 21 community programs.
The Program Participant will leave the program when the program participant, the family, the staff team and associated service providers determine that the participant:
- Demonstrated social and vocational successes in the community,
- Developed a support network of friends, family and service providers,
- Accomplished the goals set out in the ICAP,
- Articulated the need for a new challenge,
- Is community safe and aware
